Required Information for Athletic Participation

• You MUST HAVE a current, athletic physical examination performed by a physician prior to and athletic participation (Cannot accept physicals from a Nurse Practitioner or Chiropractor).
• All student-athletes will need to provide proof of screening for sickle cell trait as REQUIRED by the NCAA.  Most states started testing at birth for sickle cell trait in the early 90's, so pediatrician may have record of screening, if not then screening must be attained prior to participation in athletics.
• All student-athletes are REQUIRED to maintain primary health coverage that covers intercollegiate athletics and is accepted by local providers.
• All student-athletes must complete insurance/consent/medical history forms electronically through the ATS system.  Access and directions are provided in the corresponding links.
